Nurse Practitioner New

Schedule: Monday–Friday, 8:00AM–5:00PM EST

Daily Visits: 5–7 per day

Position Summary

We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive, longitudinal primary care services to patients in their homes and community-based settings, including assisted living facilities, skilled nursing facilities, and group homes. This role is vital in improving patient outcomes, supporting continuity of care, and delivering high-quality, compassionate healthcare.

Essential Functions

• Conduct in-home and community-based patient assessments, including medical history, physical exams, and medication reviews.
• Complete thorough evaluations of patient health status, including risk assessments.
• Diagnose conditions and develop individualized treatment plans aligned with evidence-based guidelines.
• Manage chronic conditions such as diabetes, hypertension, and cardiovascular disease.
• Provide preventive care, including screenings, immunizations, and lifestyle counseling.
• Educate patients on disease management and healthy living strategies.
• Serve as the patient’s primary care provider of record.
• Maintain accurate and timely EMR documentation (within 48 hours of visits).
• Coordinate care with physicians, nurses, and other healthcare professionals.
• Conduct follow-up visits to evaluate and adjust treatment plans.
• Participate in quality improvement initiatives and adhere to healthcare standards.
• Participate in after-hours on-call responsibilities as needed.
• Ensure reliable transportation to scheduled patient visits.
• Adhere to all policies, procedures, and compliance guidelines.
• Perform other duties as assigned.
  
  

Required Qualifications

• Active, unrestricted NP or PA licensure in the state of practice.
• Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant.
• Ability to obtain autonomous practice status within required state timelines.
• Preferred: 2+ years of primary care or longitudinal care experience.
• Active DEA license.
• Current BLS certification for healthcare providers.
• Strong clinical assessment and diagnostic abilities.
• Proficiency with EMR systems.
• Valid proof of sufficient auto insurance and reliable transportation.
• Valid driver’s license (minimum 3 years without restrictions; no DUIs or felony driving convictions in the last 7 years).
• Ability to successfully pass all required background checks and screenings.
• Arizona-specific: Minimum 1 year of experience in primary care or a related field.

Physical Requirements & Working Conditions

• Ability to lift and carry up to 20 lbs.
• Ability to sit, stand, bend, kneel, and navigate various home environments.
• Ability to remain stationary for extended periods and travel between patient sites.
• Ability to interpret data, solve problems, make informed decisions, and plan effectively.
• Work performed primarily in patient homes, with exposure to variable environmental conditions.
