Position Summary
We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive, longitudinal primary care services to patients in their homes and community-based settings, including assisted living facilities, skilled nursing facilities, and group homes. This role is vital in improving patient outcomes, supporting continuity of care, and delivering high-quality, compassionate healthcare.

Position Summary
We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive, longitudinal primary care services to patients in their homes and community-based settings, including assisted living facilities, skilled nursing facilities, and group homes. This role is vital in improving patient outcomes, supporting continuity of care, and delivering high-quality, compassionate healthcare.
Essential Functions
- Conduct in-home and community-based patient assessments, including medical history, physical exams, and medication reviews.
- Complete thorough evaluations of patient health status, including risk assessments.
- Diagnose conditions and develop individualized treatment plans aligned with evidence-based guidelines.
- Manage chronic conditions such as diabetes, hypertension, and cardiovascular disease.
- Provide preventive care, including screenings, immunizations, and lifestyle counseling.
- Educate patients on disease management and healthy living strategies.
- Serve as the patient’s primary care provider of record.
- Maintain accurate and timely EMR documentation (within 48 hours of visits).
- Coordinate care with physicians, nurses, and other healthcare professionals.
- Conduct follow-up visits to evaluate and adjust treatment plans.
- Participate in quality improvement initiatives and adhere to healthcare standards.
- Participate in after-hours on-call responsibilities as needed.
- Ensure reliable transportation to scheduled patient visits.
- Adhere to all policies, procedures, and compliance guidelines.
- Perform other duties as assigned.
- Active, unrestricted NP or PA licensure in the state of practice.
- Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant.
- Ability to obtain autonomous practice status within required state timelines.
- Preferred: 2+ years of primary care or longitudinal care experience.
- Active DEA license.
- Current BLS certification for healthcare providers.
- Strong clinical assessment and diagnostic abilities.
- Proficiency with EMR systems.
- Valid proof of sufficient auto insurance and reliable transportation.
- Valid driver’s license (minimum 3 years without restrictions; no DUIs or felony driving convictions in the last 7 years).
- Ability to successfully pass all required background checks and screenings.
- Arizona-specific: Minimum 1 year of experience in primary care or a related field.
- HEDIS/STAR measures experience
- Home-based care experience
- Knowledge of HCC documentation
- Ability to lift and carry up to 20 lbs.
- Ability to sit, stand, bend, kneel, and navigate various home environments.
- Ability to remain stationary for extended periods and travel between patient sites.
- Ability to interpret data, solve problems, make informed decisions, and plan effectively.
- Work performed primarily in patient homes, with exposure to variable environmental conditions.
- Self-motivated and able to work independently.
- Strong computer proficiency, particularly with EMRs.
- Understanding of CMS and ACA risk adjustment models.
- Strong verbal and written communication skills.
- Ability to demonstrate empathy and maintain patient trust.
- Strong decision-making skills while adhering to policies and clinical guidelines.
- Ability to adapt to changing conditions and priorities.
- Positive, collaborative approach with patients and clinical teams.
- Ability to identify, anticipate, and resolve concerns for members, providers, and health plans.
- Strong clinical abilities for performing in-home assessments.
